Interim Caregiver Service (ICS) is a government subsidised service that helps individuals who have been recently discharged from hospital. ICS is managed by the Agency for Integrated Care (AIC) and Homage is an approved provider.
ICS provides home care before long-term care arrangements are in place. This includes assistance with activities of daily living (ADLs), care tasks, vitals monitoring, medication administration and medical escort.
To apply for ICS, you will require a referral from discharge nurses and medical social workers from the hospital. Under ICS, you can receive up to 12 home visits over a 2-week period (Mondays - Saturdays) with day shifts from 8am to 8pm, and night shifts from 10pm to 8am. Any extension beyond the standard duration will require AIC's approval.
